Psychosocial Adjustment to Disease in Individuals with Diabetes

INTRODUCTION: The study has been conducted descriptively in order to determine the psychological adjustment of the diabetic patients to the disease.
METHODS: The sample of the study consists of 122 patients who are inpatients in a university hospital in Ankara and who reported to the polyclinic of the same hospital. The research data has been collected by using the ''Patient Socio-demographic Data Sheet'' and ''Psychosocial Adjustment to Illness Scale-Self Report (PAIS-SR)'' and ''The Multidimensional Scale of Perceived Social Support''. Mann Whitney U Test, Kruskal Wallis Test and Spearman Correlation Coefficient have been used as well the descriptive statistics in the evaluation of the research data.
RESULTS: As the result of the study, it has been determinated that diabetic patients’ psychosocial adjustment score mean is 45±19.1. Orientation to health care, vocational environment, sexual relationships and psychological distress have been determineted as the areas where the psychosocial adjustment is mostly affected (p<0.05). Patient’s adjustment to illness has been determineted that %34.4 good adjustment, %29.5 medium adjustment and %36.1 bad adjustment. Patient’s educational status, patient’s job, range of coming to doctor control, diet and exercise situation affect the psychosocial adjustment (p>0.05). The age of the patient, diabetic year and oral antidiabetic use in addition to insulin do not affect the psychosocial adjustment (p>0.05). It has been found that as the social supports of the diabetic individuals increase, their total psychosocial adjustment as well as adjustment to the fields of orientation to health care, domestic environment, extended family relationships, social environment and psychological distress.
DISCUSSION AND CONCLUSION: In the result of the study, it has been determineted that psychosocial adjustment score of patients with diabets mellitus is medium.

Diyabetli Bireylerde Hastalığa Psikososyal Uyum

GİRİŞ ve AMAÇ: Araştırma, diyabetli bireylerin hastalığa psikososyal uyumunu belirlemek amacıyla tanımlayıcı olarak yapılmıştır.
YÖNTEM ve GEREÇLER: Araştırmanın örneklemini, Ankara’da bulunan bir üniversite hastanesinin endokrinoloji ve metabolizma hastalıkları servisinde yatarak tedavi gören hastaları ile aynı hastanenin polikliniğine başvuran toplam 122 hasta oluşturmuştur. Araştırma verileri, araştırmacı tarafından literatür taranarak geliştirilen "Hasta Tanıtım Formu", "Hastalığa Psikososyal Uyum–Öz Bildirim Ölçeği" ve "Çok Boyutlu Algılanan Sosyal Destek Ölçeği" kullanılarak toplanmıştır. Araştırma verilerinin değerlendirilmesinde tanımlayıcı istatistiklerin yanı sıra Mann Whitney U Testi, Kruskal Wallis Testi ve Spearman Korelasyon Katsayısı kullanılmıştır.
BULGULAR: Araştırmaya katılan diyabetli bireylerin hastalığa psikososyal uyum puanı ortalaması 45±19.1’dir. Psikososyal uyumun en çok etkilendiği alanların sağlık bakımına oryantasyon, mesleki çevre, cinsel yaşam ve psikolojik baskı alanı olduğu belirlenmiştir (p<0.05). Hastaların %34.4'ünün iyi uyum, %29.5'inin orta uyum ve %36.1'inin kötü uyum sağladığı belirlenmiştir. Hastanın öğrenim durumu, mesleği, kontrole gelme sıklığı, diyet ve egzersiz durumu psikososyal uyumu etkilemektedir (p<0.05). Hastanın yaşı, diyabet yılı, insüline ek olarak oral antidiyabetik kullanımı psikososyal uyumu etkilememektedir (p>0.05). Diyabetli bireylerin sosyal destekleri arttıkça toplam psikososyal uyumlarının ve sağlık bakımına oryantasyon, aile çevresi, geniş aile ilişkileri, sosyal çevre, psikolojik baskı alanlarına uyumlarının arttığı belirlenmiştir.
TARTIŞMA ve SONUÇ: Araştırmanın sonucunda, diyabetli bireylerin hastalığa psikososyal uyumlarının orta düzeyde olduğu belirlenmiştir.
